The joke is supposed to be that A doesn't have certain anatomical parts that B does and thus would supposedly be a poor lover. The quandry, however, is resolved easily; there is more to sex than intercourse, and I'd rather receive oral sex from A than B, wouldn't you?

Yeah, I was hesitating for a moment too. It is strictly speaking the bottom part I'd most want to have sex with, but not if the top part isn't there. And particularly not if it consists of big staring fish eyes and a gasping fish mouth.
